Consent Form Completion and Submission
Email the completed NerivioTM consent form to support@nerivio.co.za

The Mi-Free Team Contacts you/the Patient
Upon receipt of the consent form the Mi-Free support team will contact you

Device Payment & Delivery
Upon payment being made the device will be delivered to your door

Patient Training (Optional)
Upon receipt of your device a nurse educator from the Mi-Free Support Team will contact you to facilitate a detailed device training session.
